Paperwork Required to Adopt from Ethiopia

Listed below are the documents that will be included in your dossier, which is submitted to the Ethiopian Ministry of Women’s Affairs, if you decide to adopt from Ethiopia. Please do not begin gathering and/or preparing these documents until you receive our Ethiopia Dossier Guide, which provides detailed instructions and sample forms to ensure that all documents are correct. We will provide you our Ethiopia Dossier Guide and Dossier Checklist immediately upon receiving and approving your completed Application for Adoption and Adoption Services Agreement.

  1. Cover letter **
  2. Letter to Ministry of Women's Affairs
  3. Home Study with Agency License *
  4. Certificate from Parent(s) regarding Post-Adoption Reports
  5. Power of Attorney (4) *
  6. Birth Certificate(s)
  7. Marriage License
  8. Divorce Decree, if applicable
  9. Medical Letter(s)
  10. Employment Letter(s)
  11. Attestation & Declaration of Personal Conditions
  12. Bank Statement
  13. Most recent Federal 1040 tax return
  14. Police Clearance
  15. Three (3) Letters of Reference
  16. signature/Photo Page of Passport (2)
  17. Two (2) passport-sized photos of each adopting parent
  18. Photographs of the adopting family (6-10)
  19. Advance Approval Form I-171H from USCIS

All documents must be notarized (or certified by an appropriate state court or agency).

* These documents must be certified by the Secretary of State from the state the document originated. Specific instructions for this procedure are given are given in the Ethiopia Dossier Guide.

** FTIA will provide these documents to you for your dossier.

Statement of Understanding and Acknowledgement & Agreement

After adopting parents send in the FTIA Application and Adoptions Services Agreement, they will be asked to sign and return the FTIA Ethiopia Statement of Understanding. The Statement of Understanding is another confirmation of your understanding of the risks of international adoption. It is not meant to alarm adopting parents, but to confirm your understanding of the process and that you are proceeding with your eyes wide open.

When accepting the referral of a child all adopting parents are required to sign an FTIA Acknowledgement & Agreement in addition to any acceptance paperwork submitted to the foreign country. This Acknowledgement & Agreement confirms your decision to complete the adoption of the child referred to you and reiterates the post placement requirements once you bring the child home.
